    Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes eight primary care sites, 40 school-based health centers and dental clinics, four day care centers, the nation's largest dental residency program, and New York State's largest behavioral health program. The network also includes 12 community medicine sites providing care to disenfranchised New Yorkers, comprehensive HIV services, chemical dependency programs, and a family support center that offers educational, vocational, and other social support programs.     Position Summary:
    We have an exciting opportunity to join our team as a Counselor - Fulltime - Monday - Friday - 9am - 5 pm - Brooklyn.

    In this role, the successful candidate The counselor is responsible for connecting eligible dental patients to outpatient health care services, as well as screening for and addressing social determinants of health.

    Job Responsibilities:

    * Engage dental paitents at clinic to assess for needs
    * Screen patients for social determinants of health
    * Identify patients that can be connected to outpaitient health care services and assists in connecting to those services. 
    * Screen patients for eligiility for public benefits and for community programs and resources
    * Assist patiens with benefit applications
    * Assists paitents in overcoming barriers to accessing health services
    * Make referrals to internal and external programs that provide food, shelter, clothing,, legal, educational, and other services.
    * Complete documentation in EPIC as required.
    * Attend staff meetings and/or clinic huddles and.any other required meetings.
    * Counsels patients on available resources and provides advocacy
    * Strengthens relationships with community partners to identify resources for patients
    

    Minimum Qualifications:
    To qualify you must have a Bachelor's Degree preferred.
    1+ years relevant experience working, ability to plan and manage,
    Knowledge of public benefits
    History of immigrant community engagement

    Preferred Qualifications:
    Excellent communication skills, ability to multi-task, experience in providing services to diverse community, ability to be a team player. Experience in working in health clinic or hospital preferred.

    Qualified candidates must be able to effectively communicate with all levels of the organization.

    NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $41,137.15 - $47,700.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
