    POSITION SUMMARY: We have an exciting opportunity to join our team as a Social Worker. In this role, the successful candidate will provide psychosocial assessment and counseling services, including education and guidance regarding home needs, planning, and support to Parkinson's disease and movement disorders patients through the NYU Langone Marlene and Paolo Fresco Institute Movement Disorders outpatient program. The LCSW will work closely with physicians, the nurse, and other clinical/non-clinical staff on a daily basis, assisting as necessary to address the social needs of patients within NYU Langone Health to ensure they have a high-quality experience throughout their care process.

    JOB RESPONSIBILITIES:

    • Provides timely and effective psychosocial assessment and evaluations as required. Uses clinical knowledge of psychosocial dynamics and interviewing techniques to gather social history, define problems and develop a relevant treatment plan.
    • Conduct patient assessments to identify emotional, social, and environmental barriers related to their treatment and/or life situation.
    • Participates in interdisciplinary planning and unit/based interdepartmental meetings.
    • Collaborate with the health care team and outside agencies, such as law enforcement, social service providers and partner organizations, and faith-based groups to provide social services to patients as needed.
    • Maintain a working knowledge of relevant medical/legal issues and social services that impact on patient care.
    • Maintain active regularly scheduled support groups. Conducts orientation patient/family groups, educating and/or elaborating on information about diagnosis, treatment
    • Facilitate the care plan across a multidisciplinary care continuum, which includes interacting with consulting physicians, other health care providers, and third-party payers to facilitate movement of the patient through the system.
    • Involve the patient in the development and implementation of their care plans which includes but is not limited to creating a series of steps that can yield immediate tangible results by building upon the patients’ strengths and changing views on perceived weaknesses and challenges.
    • Provide crisis oriented psychotherapy and brief, short-term and long-term psychotherapy and psychotherapeutic treatment to individuals, couples, families and groups, habilitation, psychoanalysis and behavior therapy; all for the purpose of preventing, assessing, treating, ameliorating and resolving psychosocial dysfunction with the goal of maintaining and enhancing the mental, emotional, behavioral, and social functioning and well-being of individuals, couples, families, small groups, organizations, communities and society.
    • Develop and implement appropriate assessment-based treatment plan; an integrated plan of prioritized interventions based on the diagnosis and psychosocial assessment of the client, to address mental, emotional, behavioral, developmental and addictive disorders, impairments and disabilities, reactions to illnesses, injuries, disabilities and impairments, and social problems.
    • Assists in the orientation of new staff or staff transferring to a new assignment. When qualified (i.e., three years post-graduate, SIFI eligible) provides supervision to social work interns.
    • Prepares statistical documentation of work in assigned areas. Demonstrates skill in presentation of psychosocial information in interdisciplinary rounds, meetings and Rusk evaluation conferences.
    • Is competent to care for patients of all age groups.
    • Performs clinical duties in an independent manner
    • Documents all social work activities in a timely manner according to departmental protocols.
    • Mobilizes community agencies and other support networks in the interest of patients/families.
    • Partnering with Care Manager to ensure effective throughput.

    MINIMUM QUALIFICATIONS: To qualify you must have a Masters degrees in social work. Active licensed Social Worker (LCSW). Minimum of one year of progressively responsible related experience. Proficiency in using various Microsoft Office applications such as Word, Excel, Access, Power Point and Outlook. Familiar with internet applications. Strong oral, written, communication, interpersonal skills. Ability to interface effectively with all levels of management and must work and communicate effectively with both internal and external partners. Ability to work within a team environment as well as independently. Time management skills and ability to work well under pressure. Ability to multi task and project manage. Works under general direction.. REQUIRED LICENSES: Licensed Social Worker - NYS

    PREFERRED QUALIFICATIONS: Minimum of 2 years of experience in social work and broad background with diverse populations in a medical facility with neurological or psychiatric needs preferred.

    NYU Langone Health provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $27.41 - $37.72 Hourly. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
