    Position Summary:
    We have an exciting opportunity to join our team as a Pediatric Certified Diabetes Care and Education Specialist Outpatient Pediatrics.

    In this role, the successful candidate provides comprehensive diabetes self-management education and training to individuals and groups according to the scope of practice, and standards of professional performance for Diabetes Educators for patients at the Robert I Grossman MD, and Elisabeth J. Cohen, MD, Pediatric Diabetes Center and patients admitted to NYU Langone Orthopedic Hospital/RUSK and Hassenfeld Children's Hospital at NYU Langone. Responsibilities include supporting the diabetes practice, being an active member of the interdisciplinary team, comprehensive diabetes education for patients, families, caregivers and staff, collection of blood glucose data and the surveillance of outcome measurements, nutrition therapy, maintenance of evidence-based hypoglycemia and hyperglycemia management order sets and protocols (as well as monitoring, tracking, and root cause analysis to prevent errors and patient harm), individualized medication management plan, plan of care that facilitates smooth transition across care settings.

    Job Responsibilities:

    * Quality initiatives and performance development and implementation utilizing evidence based practices
    * Support physicians care for all pediatric diabetes center patients living with all types of diabetes and pre-diabetes through appropriate interventions and education to achieve all identified treatment goals.
    * Participates in clinical co-management with physicians and other care providers. This includes providing input into patient education, identifying barriers to care, care coordination and transition, making appropriate modifications of diabetes medical regimes, per protocols, based on critical assessment of patient clinical factors and blood glucose data.
    * Establish policies, and protocols and standards of practice in accordance with best practice. Development and implementation of the diabetes education curriculum, assist in planning diabetes center events.
    * Encourages research to validate practice and improve quality Diabetes Self Management and Training (DSME/T)
    

    Minimum Qualifications:
    The Certified Diabetes Educator (CDE) is a health professional (RN/ RD/ NP/ PA) who is either registered or licensed by an appropriate accrediting body in his/her respective field with a minimum of a bachelor's degree, masters preferred. The CDE must be certified by the National Certification Board of Diabetes Educators. Basic Life Support for Healthcare Providers.
    The CDE should have a background in pediatrics with a minimum of one year of diabetes education either in hospital setting or outpatient practice. Specific skills should include clinical competence in the following: use of diabetes medications including insulin, clinical management of all patients with diabetes, all aspects related to education and successful use of continuous subcutaneous insulin pumps and continuous glucose monitoring, management and modification of individual diabetes plan of care based on optimizing glycemic control and specified targets..

    NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $81,325.14 - $110,361.24 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
