POSITION SUMMARY: We have an exciting opportunity to join our team as a Social Worker within Abdominal Transplant. In this role, the successful candidate Under the Supervision of a Social Work Supervisor provides psychosocial assessment and counseling services, including discharge planning, to patients as assigned by the Department of Social Work. This Social Worker will be an integral member of an interdisciplinary adult solid organ abdominal transplant team, conducting psychosocial transplant evaluations of potential recipients and their families to determine eligibility for transplantation, availability of social support systems, coping skills, attitudes towards transplantation and identifying possible barriers to adherence with pre and post-transplant care. This also includes lifelong psychosocial care and case management post-transplant. There are also shared patient responsibilities with NYU Langone Hospital-Long Island renal transplant candidates. The Social Worker in this role will be primarily responsible for outpatient assessment and follow up of pre-/post-transplant patients and for urgent transplant eligibility evaluations of inpatients as required.
