    POSITION SUMMARY: We have an exciting opportunity to join our team as a Sr. Health Information Management Associate (Sun 9am-5pm and Mon-Thurs 6am-2pm). In this role, the successful candidate provides customer service to the registration and scheduling areas of NYU Hospitals Center regarding the demographics on patients medical records. Assists customers by changing critical patient identifiers as appropriate. Merges duplicate medical record numbers after extensive investigation and verification. Assists clinical staff with the chart correction process.

    JOB RESPONSIBILITIES:

    • Responds to telephone or email requests from registration or scheduling areas to update patient demographics or merge medical records in Epic. Ensures that all requests are completed in a timely manner. Communicates effectively using written/verbal skills. Maintains a professional and friendly demeanor.
    • Performs other duties as assigned.
    • Assists clinicians with chart correction functions. Provides general technical directions on how to correct documentation errors within electronic patient records.
    • Communicates effectively among the Merge Center shifts and registration staff. Assists Merge Center teammates with decision making. Forwards unresolved requests to the next shift as appropriate. Communicates effectively via email. Brings concerns, issues or discrepancies to the attention of management.
    • Completes assigned potential duplicates reports as outlined in Merge Center policies and procedures. Completes assigned cases within the reports within a timely manner.
    • . Investigates demographic changes for validity and accuracy. Utilizes the audit report provided within Epic. Ensures that any changes made to demographics have supportive documentation provided. Questions any changes without provided documentation. Informs the registration staff of the information necessary to proceed with a change or merge.
    • Completes duplicate research by merging medical records in Epic or marking records as known non duplicates (KND). Follows through with decisions post-research by selecting the appropriate action. Ensures that the chosen action performed is based on evidence (three consistent identifiers). Provides notes within the record of the reasoning behind the action taken.
    • Researches possible duplicates utilizing all appropriate tools, systems and materials in accordance with the Merge Center policies and procedures. Clarifies questionable situations with the Merge Center Manager. Makes decisions based on the outcomes of research.

    MINIMUM QUALIFICATIONS: To qualify you must have a High school diploma with 2-3 years of experience in an area directly related to the position or equivalent combination of education and experience. Demonstrated thorough knowledge of Health Information Management or hospital operations is required. Knowledge of medical terminology and computer skills is required. Demonstrated excellent verbal and written communication and interpersonal skills required.

    PREFERRED QUALIFICATIONS: Associates or Bachelors Degree in Health Information Management or Technology.
