    We have an exciting opportunity to join our team as a Pharmacy Utilization Management Coordinator.

     

    Position Summary:

     
    This position contributes to improved health outcomes for patients by partnering with NYUPN Physicians and attributed patients to advise and educate on issues regarding medication, and drive strategies that address pharmacy utilization across the network.

     

    Job Responsibilities:

     

    • Ensures compliance with all regulations.
    • Develops and documents clinical interventions for areas for improvement across medication appropriateness, adherence and compliance.
    • Seeks professional development opportunities and participates in projects/activities to maintain professional competence (seminars, journals, 15 hours of continuing education credit per year required by NYS).
    • Coaching, mentoring and precepting pharmacy students and residents.
    • Demonstrates knowledge of the organization's Service Standards and incorporates them into the performance of duties.
    • Assists physicians with individual clinical decision making as requested.
    • Maintains a close relationship to the Pharmacy & Therapeutics committee, reporting outcomes, data and trend analyses to both the Clinically Integrated Network and the committee.
    • Visits practices on regular basis to discuss data and trends.
    • Collaborates with the registered nurse Clinical Care Coordinators to refer patients for health coaching as well as receive referrals and perform medication coaching.
    • Partners with prescribers to improve utilization by providing recommendations and education.
    • Develops strategies and targeted interventions to manage high cost drug utilization and improve prescribing practices.
    • Reviews charts and works with physicians and health plans to review medications for appropriateness, duplications, adherence and compliance, while consulting with patients as necessary.
    • Performs other duties as assigned.

     

    Minimum Qualifications:

     

    • Doctor of Pharmacy
    • Post Graduate Year 1 Residency Training
    • NYS Pharmacy License and Registration
    •  3+ years of Pharmacist experience
    • Effective oral and written communication and presentation skills
