    ACCOUNTABILITY:  Facilitates the development of standards that support nursing care for the patient with a stoma, wound, continence and/or patients at risk for or have an actual pressure injury. Coordinates evaluation of technology and new products that facilitate care of the patient with a skin breakdown or at risk for same.  Mentors the Skin Care Resource Nurses; leads and coordinates activities of the Skin Care Council. Acts as a liaison to the Nursing Education Department in identifying educational needs of staff concerning issues with skin care or "ostomy" patients. Provides oversight for the VAC and Specialty Bed Rentals and provides consultation for appropriate utilization.  Provision, coordination, and assurance of comprehensive nursing care using the nursing process and pertinent standards of care, the assessment and identification of illness and physical conditions, and performance of therapeutic, corrective, and recommendations of the designated inpatient or outpatient patient population.

     

    REPORTABILITY: Chief Medical Officer, the Director of Advanced Practice Nurses and Physician Assistants and Vice President of Nursing. Organizationally reports through the Vice President of Nursing and Director of Advanced Practice Nurses and Physician Assistants.  Additionally collaborates with the Chief Nursing Officer, the Senior Vice President, Directors of Nursing, other nursing leaders, Social Work and Care Management.

     

    REQUIREMENTS:

    Licensure: Current registration as a Professional Nurse in New York.

    Education: Bachelor’s Degree in Nursing; a graduate or enrolled in an approved accredited WOCN program (or WOCNCB certified via experiential pathway).

    Certification: Required certification by the Wound, Ostomy, and Continence Nursing Certification Board (WOCNCB) (within 1 year of employment).  BLS certified.  PRI Certification/Screening is optional.

    Experience:

    • At least four years of clinical nursing practice in the care of the patient population served; ability to apply nursing process toward achievement of specific outcomes in an organized fashion, which addresses patient/family safety and
    • Previous experience as a WOC nurse is
    • Evidence of excellent interpersonal skills, effective communication skills, creative problem solving and excellent critical thinking and
    • Previously demonstrated ability to facilitate group processes and work cohesively and collaboratively as member of the interdisciplinary
    • Effective change
    • Physical stamina, manual dexterity, visual and aural acuity to perform
    • Membership in professional specialty organizations.

     

    Job Knowledge: Evidence of excellent interpersonal skills, effective communication skills, and creative problem solving; understands the domains of the Magnet Model and implementation strategies pertinent to this specialty area of practice.

    Consistent evidence of ability to effectively work as a consultant to all levels of staff.

    Previous demonstration of ability to facilitate group processes and work cohesively with an interdisciplinary team. Physical stamina, manual dexterity, visual and aural acuity to perform responsibilities.

     

     

    RESPONSIBILITIES:

     

    1. CLINICAL

     

    1. Performs complete health history, physical examination and psychosocial assessment of patients within the designated specialty
    2. Provides clinical leadership in assessing, implementing and evaluating the care provided to wound, ostomy, and continence patients and their significant others according to professional and departmental
    3. Makes recommendations for the appropriate laboratory, radiological and diagnostic studies per findings specific to the Wound, Ostomy, and/or Continence patient population if consistent with privileges.
    4. Recommends medical equipment and devices as they relate to the Wound, Ostomy, and/or Continence patient population if consistent with privileges.
    5. Provides clinical leadership in assessing, implementing and evaluating the care provided to wound, ostomy, and continence patients and their significant others according to professional and departmental standards.
    6. Assumes responsibility for clinical operations of the wound, ostomy, and continence nursing service/program.
    7. Acts as a professional role model to all levels of staff by incorporating expertise and related experience into all aspects of patient care and the nursing process.
    8. Facilitates a strong collaborative interdisciplinary approach to achieve optimal patient outcomes.
    9. Develops, promulgates, and fosters the implementation of process standards for the delivery of patient care relevant to area of practice.
    10. Fosters ongoing patient/significant other education in an effort to facilitate effective clinical management across the continuum of care consistent with the individual needs of the patients and goals of the institution.

    2. Promotes patient, physician, and other consumer satisfaction by facilitating excellent customer service by all members of the health care team.
    3. Provides leadership for management of the in- and outpatient wound, ostomy, and continence service serving as a role model and resource for staff.
    4. Serves as a role model for staff in considering needs and behaviors of specific patient age groups.
    5. Assures compliance with standards and regulations promulgated by all applicable agencies.
    6. Provides a suitable plan for optimal availability of resources for safe, competent, and effective patient care delivery.
    7. Practices prudent fiscal management of the wound, ostomy, and continence nursing service.

     

    EDUCATION/DEVELOPMENT

     

    1. Promotes own professional growth, development, and expertise in the role.
    2. Creates unit milieu supportive of the education and development of the nursing staff and other health care providers consistent with the goals of a learning organization.
    3. Identifies and addresses learning needs of professional and ancillary staff members.
    4. Utilizes resources to plan and meet unit and individual staff learning needs.
    5. Evaluates the effectiveness of education and development activities in achieving desired outcomes.
    6. Serves as support and resource for the education of other health professionals.

     

    1. EVALUATION AND RESEARCH

     

    1. Develops system for identifying, tracking, and managing patients in need of wound, ostomy, and continence services.
    2. Contributes to the development of systems for collecting and analyzing data relevant to program evaluation.
    3. Monitors practice patterns for trends in outcomes, costs, and resource utilization for the wound, ostomy, and continence population.
    4. Leads or supports quality and performance improvement initiatives consistent with departmental programs.
    5. Develops and implements action plans in response to internal and external benchmarking data, problems, findings, issues, incidents, and complaints.
    6. Leads or participates in interdisciplinary quality improvement programs and cultivates environment that promotes patient/significant other, physician, and staff satisfaction.
    7. Applies research findings that support development of evidence-based practice.
    8. Participates in and fosters staff involvement in nursing research efforts.
