    Position Summary:
    We have an exciting opportunity to join our team as a Supervisor, Social Work - LCSW - Acute Care/Hospital - Mineola.

    In this role, the successful candidate is responsible for the organization, administration and monitoring of the processes and procedures necessary for the operation of the Social Work Department. Ensures the Social Workers provide psycho-social support services, in accordance with professional standards and practice, to patient/family experiencing issues in social and/or emotional adjustment. Utilizes professional background and skills to facilitate patient-family meetings and assists team, attending physicians, staff, and patient and family in the clarification of the goals of care and the plan of care. Collaborates with the multidisciplinary team, both internal and external to the organization advocating for the patient/family to ensure smooth discharge planning and clear follow-up post discharge. Supervises the Social Work Department team in ensuring excellent patient care services, coordinating educational and outreach services, leading quality improvement initiatives, and linking team efforts with other hospital departments and with outpatient and community services.

    Job Responsibilities:

    * Completes psychosocial assessment as appropriate. Develops and implements Social Work Intervention Plan to address needs of the patient.
    * Utilizes the Departmental High Risk Screening Criteria to identify admissions with potential Social Work intervention needs.
    * Actively participates in multidisciplinary rounds on the patient care units. Communicates psychosocial needs of patient, with intervention plan of care and goals, to the multidisciplinary team. Coordinates patient family conferences as necessary to facilitate communication regarding treatment plans and decision making.
    * Oversees screening of patient admissions for potential clinical social work needs. Oversees assessment of patients meeting departmental screen criteria. Supervises ongoing social work intervention activities.
    * Formulates and recommends plans to the Director, Social Work regarding future program development in accordance with current clinical professional standards of practice with regard to clinical social work. Oversees the recruitment, selection, scheduling and evaluation of staff members. Makes decisions regarding discipline of staff in collaboration with Director, Social Work.
    * Provides counseling and support services to each patient designee experiencing difficulty in adjusting to an altered physical status andor changing psychosocial needs.
    * Develops indicators relative measurement of patient care quality. Utilized indicators for quality measurement. Evaluates outcome of measurement.
    * Performs other duties as assigned
    * Collaborates with the Director, Social Work in the development of policies and procedures of the Social Service Department regarding clinical social work. Evaluates and revises policies and procedures as necessary according to changes in standards of care. Participates in development and implementation of appropriate measures of patient care quality and satisfaction.
    * Coordinates in-service educational updates as necessary for staff development. Provides educational services as necessary to other disciplines regarding the Social Service Department. Obtain and maintain CIFI certification and encourage eligible staff to do the same. Represent s Social Service Department at the quarterly Nassau County Adult Protective Services Meetings. Collaborates with Supervisors of Case Management and Winthrop Hospital attorneys to advocate for patient and their rights including the right to self-determination.
    

    Minimum Qualifications:
    To qualify you must have a Master's Degree Social Work. Current NYS Clinical Social Worker License (LCSW). 3 - 5 years Post Master's Degree experience in Health or social agency. Excellent communication (both oral and written), interpersonal, organization, leadership, and problem solving skills. Ability to provide assistance based on physical, psychosocial, educational, safety and related criteria, appropriate to the age of the patients served. Thorough understanding of social service operations within the acute care hospital.

    Required Licenses: Lic Clinical Social Worker (LCSW)

    Preferred Qualifications:
    Hospital Social Work Experience

    NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $77,258.68 - $128,764.47 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
