    Job Description

    POSITION SUMMARY: The Project for Assistance in the Transition from Homelessness (PATH) outreach worker seeks out, interviews and assist persons who are homeless and mentally ill by linking these individuals to appropriate housing and mental health services. The outreach worker will work in a variety of settings in five days a week including non-traditional hours including looking for people in areas not meant for habilitation: parks, wooded areas, abandoned homes. 

    SUPERVISORY RESPONSIBILITIES: None

     

    ESSENTIAL JOB FUNCTIONS:

     

    ·       Conducts outreach with homeless individuals identified in various community settings: streets, shelter, day resource centers, library, etc.

    ·       Assesses and documents individuals for program eligibility. (i.e. homelessness, mental health disorders).

    ·       Provides information and referrals to community resources, including mental health, AODA, medical, educational, financial services, based on assessment.

    ·       Follows up to ensure that clients are connected to and utilize referral services.

    ·       Advises clients regarding immediate needs, such as food and shelter.

    ·       Identifies and responds in emergency situations (mental health, physical, weather-related, etc.).

    ·       Assists individuals who are applying for Social Security Disability and other public benefits.

    ·       Transports and accompanies clients to appointments, using public transportation or personal automobile, as needed.

    ·       Assists in housing placement for displaced individuals.

    ·       Maintains case records/files using the standard forms.

    ·       Maintains case records in a timely manner.

    ·       Ensures case records are available for generating reports, audits, grant writing, supervision.

    ·       Enters all necessary client data into HMIS.

    ·       Uses Case Management tool to track statistical and geographical information which is not entered into HMIS for monthly reporting.

    ·       Submits and reviews reports with supervisor.

    ·       Consults with supervisor regarding problems or concerns.

    ·       Attends weekly program meeting and other meetings as directed by the supervisor.

    ·       Other duties as assigned.

     

    ADDITIONAL JOB FUNCTIONS:

     

    ·       Coordinates with other Neighborhood Alliance programs to support participants
