    Job Description

    QIDP/QMRP SUPERVISOR PROGRAM IDD

    As a mission-driven organization, we are intensely focused on expanding access to services and developing new and more effective ways of delivering supports and services to individuals we serve.

    REM Nevada is seeking a QIDP to provide case management reviews and case supervision. This individual will supervise assigned staff and support the development of and oversee the implementation of service plans. 

     

    KEY RESPONSIBILITIES: 

     

    • Responsible for the supervision of client cases within the assigned management team. Reviews service plans to ensure completeness_._
    • Provides direction, support and counsel to host family on a weekly basis. Involves treatment team in decisions relating to changes in treatment plan and arranges additional support as necessary.
    • Interviews clients to obtain medical history data and other pertinent information.
    • Provides intensive case management services, coordinating both intra- and inter- agency services, to ensure on-going assessment and follow-through of treatment plans.
    • Plans therapeutic treatment such as behavior modification, social skills, anger management, and stress management therapy.
    • Develops and implements Person Centered Plans (PCP) including annual and ongoing meetings.
    • Develops and implements Behavior Plans.

    Qualifications:

    • Bachelor’s Degree in Psychology, Social Work, Education or other Human Service field

    • At  least one year of experience working directly with persons with intellectual disabilities

    • Minimum of one year experience as a Case Manager.

    • Valid NV Driver's License

    • Successful clearance of background including fingerprints and TB screen

    • Other education, experience, and licensure as required by applicable regulations and state

    • Current CPR/First Aid Certification as required by state

    • Strong attention to detail, organizational skills, and the ability to multi-task to meet deadlines

    • Effective communication skills to manage relationships
