    Job Summary

    New Horizon Services is seeking a dedicated and compassionate Agency Nurse specializing in medication management to join our team. The nurse will play a pivotal role in providing high-quality care and medication management services to individuals with intellectual and developmental disabilities (IDD). The ideal candidate will be committed to improving the health and wellbeing of our clients through person-centered planning, clinical oversight, and adherence to best practice guidelines.

    Essential Duties and Responsibilities

    • Medication Management : Implement and oversee medication management programs according to DDNA's best practice guidelines. Ensure the safe administration and documentation of medications in compliance with state laws and regulations.
    • Evaluation and Oversight : Provide initial and ongoing evaluations of clients' medication management needs. Develop individualized plans of care that promote each client's independence and dignity to the greatest extent possible.
    • Education and Training : Develop and deliver comprehensive training programs for unlicensed staff involved in assisting with medication management, ensuring consistency and competency in medication-related tasks.
    • Collaboration and Coordination : Work closely with an interprofessional team, including healthcare providers, support staff, and family members, to ensure a coordinated approach to medication management and care planning.
    • Policy and Procedure Development : Assist in the development and implementation of policies and procedures related to medication management, ensuring adherence to best practices and regulatory compliance.
    • Quality Improvement : Participate in quality improvement initiatives related to medication management. Identify and implement strategies to prevent medication errors and enhance client safety.
    • Client Advocacy : Advocate for the rights of clients to direct their own care and participate in decision-making processes related to their health and medication management.

    Qualifications

    • Current Registered Nurse (RN or BSN) license in Kansas.
    • Minimum of two years of nursing experience, preferably in a community setting serving individuals with IDD.
    • Demonstrated knowledge of medication management practices and guidelines for individuals with IDD.
    • Strong communication and interpersonal skills, with the ability to educate and train staff effectively.
    • Commitment to a person-centered approach to care and support.
    • Ability to perform clinical evaluations and develop individualized care plans.
    • Proficiency in electronic health records and medication management software.

    Physical Requirements

    • Ability to perform physical tasks associated with nursing care and medication administration.
    • Must be able to work flexible hours, including some evenings and weekends, to meet client needs.
