    Case Manager for Homeless Families

    ESSENTIAL JOB FUNCTIONS:

    Assist with the application and assessment process for new applicants of the program

    Complete biopsychosocial and needs assessments to determine appropriate goals for residents

    Create long-term and short-term case plans with SMART goals that will meet the family’s individual needs and lead to self-sufficiency

    Facilitate life skills and job-readiness training to assist residents in meeting their goals

    Coordinate the referral process for additional services for residents

    Hold resident’s accountable for making progress towards their goals

    Conduct individual case management sessions

    Communicate frequently with residents (including some calls/texts after hours)

    QUALIFICATIONS

    Ability to provide grace-centered biblical spiritual direction

    Undergraduate degree in social work or related field or equivalent work experience

    Minimum of 1-year successful experience in adult case management or similar position

    Strong organizational, verbal and written communication skills

    Ability to work well with a team

    Ability to utilize Microsoft Office applications (Outlook, Word, Excel)

    Ability to write clear, accurate case notes and complete other required case documentation thoroughly and accurately

    Must maintain professional behavior to include respecting residents, maintaining confidentiality and setting appropriate personal boundaries

    Must have dependable vehicle, valid driver’s license and a clean driving record

    SALARY/HOURS/ETC.

    Full-time position – 40 hours per week, mostly Monday – Friday and some Saturdays

    Salary dependent on work experience, skill set, and education

    Drug Testing and Background Check required
