    POSITION SUMMARY: The Diabetes Program Coordinator within the Nursing Staff Development Unit in Office of School Health will be responsible for supporting the Diabetes Program. Performs related work.

     

    REPORTS TO:  Diabetes Supervisor of Nurses

     

    DIRECT REPORTS: N/A

     

    KEY RELATIONSHIPS: Works closely with the Diabetes Supervisor of Nurses and the Diabetes Staff Nurse Educator as well as school staff: nurses, supervisors, borough nursing directors, providers, and others as necessary.

     

    RESPONSIBILITIES

     

    • Plans, coordinates, and communicates important information regarding the Office of School Health’s diabetes program to nurses, school staff, parents, and clinical staff.
    • Serves as liaison between OSH and other organizations regarding diabetes management in schools.
    • Networks with primary care providers and endocrinologists, and diabetes pump representatives to support and enhance school diabetes services.
    • Participates in DOE, DOHMH, and OSH public events aimed to foster community awareness of OSH programs and services.
    • Coordinates trainings and supports to schools related to diabetes care.
    • Assists in preparing training and professional development by coordinating venues and schedules for OSH diabetes staff and school staff.
    • Organizes and monitors schedules, outcome, and follow-up of diabetes trainings. Maintains database of requests for training and support.
    • Provides technical assistance to relevant staff to enable proper management and implementation of diabetes care.
    • Assists in the data entry, review, and analysis of diabetes statistical data for tracking purposes.
    • Assists with OSH special projects related to diabetes.
    • Researches information required for special projects and reports; ensures the completion of required reports and documentation.

     

    QUALIFICATION REQUIREMENTS:

     

    MINIMUM

     

    1. Graduation from an accredited college with a baccalaureate degree and two years of experience in community work or community centered activities in an area related to the duties described above; or
    2. High school graduation or equivalent and six years of experience in community work or community centered activities in an area related to the duties as described above; or
    3. Education and/or experience which is equivalent to the requirements in “1” and “2” However, all candidates must have at least one year of experience as described in “1” above.

     

     

     

    PREFERRED

     

    • Knowledge of diabetes.
    • Ability to effectively communicate, both orally and in writing, and to collaborate with others.
    • Excellent organizational skills.
    • Knowledge of the NYC school system and structure.
    • Strong work ethic and the ability to align work products with organizational vision and goals.
    • Proficient in Microsoft Office.
    • Internal candidates preferred.

     

    SALARY: $53,961+
