Leading New York City Social Service organization seeks a Social Worker (LMSW, LMHC) to work in the Staten Island’s Children’s Rapid Response Mobile Crisis program.

Responsibilities:

• Provide outreach to families in crisis in the community within 2 hours of receiving the referral. With the help of the Crisis Family Advocate Social Workers help to de-escalate the child in crisis as well as helping families manage emotional reactions to child’s crisis.
• Coordinate all services for consumers, including day programs, behavioral health, medical care, transportation, summer programs (when applicable), recreational activities, and community inclusion
• Serve as a liaison between the department and consumer's family members, and relevant community members.
• Maintain records for all contacts with clients and their families.
• Link clients to community resources that support their behavioral health goals.
• Conduct meetings to ensure proper treatment, progress in program and coordination of goals.

Qualifications:

• Licensure Master Social Work (LMSW) or Licensed Mental Health Counselor (LMHC)
• 1 year of experience

Salary: $60,000
