Job Description
Norco is currently seeking an individual to fill our Home Transition Coordinator position. The Home Transition Coordinator is responsible for meeting response times for patient discharge from the healthcare facilities and reflecting Norco’s image with the provider community and be an extension of customer service. Problem solving includes continual prioritization and reprioritization of customer (patient and referral source) needs. Impact includes potential for significant customer service implications.
