    Job Description

    The FILM FILE CLERK reflects the mission, vision, and values of Northwestern Memorial, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

    The Film File Clerk prepares, process and maintains radiographic film files and softcopy images for interpretation and review by physicians and other healthcare professionals regarding patient assessment, workup, therapeutic procedures, clinical course, diagnosis. Prognosis, etc., in order to document patient care and facilitate delivery of healthcare.

    RESPONSIBILITIES:

    • Processes films for interpretation
      • Locate films and on line images as requested.
      • Creates and distributes CD Rom where applicable
      • Creates pull list for films where applicable or manages on-line lists
      • Pulls previous films where hardcopy comparison is required
      • Refiles films and reports where applicable once case is read and report processed.
    • Meets established quality and productivity standards and deadlines
      • Performs job duties which are an established operating guideline.
    • Verifies patient information for accuracy and completeness
    • Purges case to off site storage.
      • Removes cases no longer active to off site storage.
      • Identifies special groups of cases as no purge cases (i.e., pediatric, mammography, legal).
    • Maintains film file area
      • Files films in proper film file insert within film file master jacket where applicable.
      • Files report copy of exam within master jacket within 48 hours or less of receipt where applicable.
    • Duplicates films as requested
      • Provides duplicate films or CD ROM to requesting Medical Staff and patients
      • Sends duplicate films to requesting Units in hardcopy environments only
      • Duplicates film file record as requested by other customers (patients, legal and other healthcare professions) in accordance with NMH policy.
    • Provides in person and telephone response to customer request for patient film file records.
      • Releases film file record information
      • Signs out film file records
      • Mails film file records
      • Provides documentation for release of records.
      • Maintains outside film logs

    Additional Functions:       

    • Demonstrates an understanding of the medico legal implications and responsibilities related to maintains of patient records to protect the patient and the institution
      • Understands and complies with policies and procedures related to medico legal matters, including confidentiality, amendment of medical records, release of information, patients' rights, medical records as legal evidence, informed consent, etc.
      • Recognizes and reports to appropriate risk management personnel, unusual circumstances and/or information with possible risk factors
      • Recognizes and reports problems, to appropriate supervisors, errors, and discrepancies in patient records
    • Operates information system as directed to complete assignments.
      • Documents imaging procedures manually and/or electronically to insure patient and film file tracking.
      • Uses designated equipment effectively, skillfully and efficiently.
      • Maintains equipment and work area as directed.
      • Assesses condition of equipment and furnishing, and reports need for replacement or repair.
      • Follows policies and procedures to contribute to the efficiency of radiology information department.
      • Organizes and prioritizes assigned work, and schedules time to accommodate work demands, turnaround-time and commitments.
      • Maintains required records, providing reports as scheduled and upon request
      • Participates in quality assurance programs
      • Provides clerical/technical support as needed and as assigned.

    • Basic knowledge of anatomy as required by area of responsibility
    • Effective communication and interpersonal skills
    • Ability to operate information system equipment and other equipment as specified
    • Ability to work under pressure time constraints.
    • Ability to communicate effectively
