The ON CALL SOCIAL WORKER reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
The On-Call Social Worker is responsible for providing high quality services to the patient and the patients family in order to promote optimal social and emotional functioning. These services include responding to Mobile Crisis Response calls and completing psycho-social risk assessments using the IM-+CAT and mental health assessment as well as providing appropriate crisis interventions and referral and linkage to other agencies/treatment facilities. The Social Worker facilitates appropriate disposition for MCR patients through collaboration and consultation with physicians and other appropriate staff. Most of these crisis evaluations occur in the Emergency Department of both Northwestern hospitals, Kishwaukee and Valley West hospital but may occur within the community.
