    Job Description

    The CMO of Population Health (“CMO”) is responsible for designing and enabling care model transformation in support of Northwestern Medicine’s Patients First Mission and value-based care contract performance. The CMO collaborates with and engages clinical and system function partners throughout NM and across the community to implement population health management programs.

    Responsibilities

    • Develops population health strategies and manages interventions, people, and programs that address the clinical and utilization needs of various risk-based populations.
    • Oversees the quality improvement, and medical management processes to improve care, reduce costs, and deliver evidence-based outcomes.
    • Lead strategy and operations for care coordination and patient outreach efforts.
    • Serve as primary liaison to specialty service lines in pursuit of utilization management strategies.
    • Partner with Primary Care Leadership to identify and implement strategies to optimize operations to support strong quality outcomes and contract performance.
    • Support the optimization of the Northwestern Medicine’s Clinically Integrated Network’s (CIN’s) quality program.
    • Develop and facilitate relevant clinician engagement and education efforts.
    • Identify and optimize appropriate resourcing for clinical population management efforts.
    • In partnership with the VBC Contracting and Performance team, develops risk stratification and population segmentation models.
    • Participate actively in the NM CMO structure to ensure coordination of Population Health efforts across ambulatory and hospital sites
  • Qualifications

    Qualifications

    Required :

    • Board-certified physician (M.D. or D.O.)
    • Experience participating in and/or implementing alternative payment care models.
    • Minimum 15 years health care experience.
    • Minimum of 8 years clinical practice.
    • Previous experience leading a team, business unit, and/or clinical improvement initiative.
    • Excellent interpersonal skills, as demonstrated by the ability to work effectively with physician partners, individuals and/or teams, across disciplines.
    • Demonstrated skill in change management.
    • Excellent oral and written communication skills.
    • Experience leading a data-driven improvement effort.

    Preferred :

    • Additional graduate education in health administration, business, public health, or a related field.
    • Experience leading Population Health programs

    Reporting structure

    • Reports directly to NM SVP for Population Health and Primary Care
    • Accountable to NM System CMO and NM System CFO
