    Job Description

    The Transplant Assistant reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

    The Transplant Assistant is a member of the transplant team who serves as a resource to patients and multidisciplinary team members. The Transplant Assistant provides support through all phases of patient care. Job duties may vary by specific department or assignment.

    Scheduling:

    • This position is regularly scheduled to work five 8-hour shifts during the day, Monday through Friday.

    _ Responsibilities:_

    • Assists patients and clinicians with prescription assistance program enrollments
    • Assists patients and clinicians with FMLA, social security paperwork
    • Assists clinicians with obtaining prior authorizations and denial appeals with insurance companies for variety of specialty medications and procedures
    • Helps patients coordinate specialty pharmacy medication deliveries
    • Collaborates and coordinates care with internal and external agencies (including but not limited to, relaying home health care orders, transfusion and infusion appointments)
    • Collaborates with clinicians to ensure expected labs and other test results are obtained
    • Performs accuracy checks then imports data from external software into patient electronic medical records
    • Assists in coordinating care of transplant donors and recipients as directed by clinical staff
    • Facilitates communication with patients, serving as a liaison between patients and clinical staff to provide the best patient experience
    • Assists in maintaining the integrity of electronic and paper medical records
    • Faxes, mails, and emails medical information to internal departments and external agencies, including but not limited to case managers, referring physicians, home health companies, Rental Network, etc.
    • Understands basic transplant clinical priorities and urgencies
    • Collaborates and provides cross-coverage with other Transplant Assistants
    • Collaborates with assigned clinical staff to complete job-specific tasks
    • Participates on committees and attends meetings as appropriate
    • Assists with preparations for regulatory audits and surveys
    • Identifies improvement opportunities and participates in quality initiatives and special projects
  • Qualifications

    Qualifications

    ** Required :**

    • High School Diploma

    ** Preferred:**

    • Experience in a Medical Setting
    • Experience in Medical Records
