    Job Description

    The SOCIAL WORKER reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

     

    The Social Worker is responsible for providing psychosocial counseling and intervention services to patients who request their services, or any patient referred by a provider. The social worker advocates for the patient and family, and assists in transitions of care and outpatient continuity of care. For each problem, issue, or concern, develops observable and measurable goals and expected outcomes in the areas of problem resolution, utilization and resource management, and patient satisfaction.

    DESCRIPTION

    ·         Completes a thorough psychosocial assessment for patients.

    ·         Utilizes critical thinking to identify and screen high risk social cases.

    ·         Provides services which may include but are not limited to pre-treatment planning, psychosocial counseling/interventions, health education, financial counseling, referrals, psychotherapy, cognitive behavioral therapy, and discharge/transition planning.

    ·         Collaborates with team, other ancillary departments, case management staff, and social service agency providers to develop a care plan.

    ·         Provides resources, including education materials, to patients and families in the areas of financial assistance, transportation, scheduling, and community resources to support the care plan.

    ·         Demonstrates knowledge of community resources and an ability to connect patients and families with these resources.  Acts as an advocate on behalf of the patient who requires assistance to gain access to needed information, resources, or services.

    ·         Supports department/organizational initiatives (attends case conferences, participates in quality improvement).

    ·         May perform other duties as assigned.

    ·         Demonstrated customer service skills, including the ability to use appropriate judgment, independent thinking and creativity when resolving customer issues.

    ·         Ability to balance multiple priorities.

    ·         Excellent verbal and written communication skills.

    ·         Ability to use personal computers and select software applications.

    ·         Ability to analyze data for decision making purposes.

    ·         Strong computer skills, including Microsoft Office, Outlook and database entry.

    ·         Ability to maintain a high degree of confidentiality

    ·         Ability to adapt to changes in work environment, delays or unexpected events.

    ·         Demonstrates attention to detail and monitors own work for accuracy.

    REQUIRED:

    ·         Licensure in Illinois

    ·        Licensed Clinical Social Worker (LCSW) 

    ·         Zero (0) to two (2) years’ experience in a relevant role.

    PREFERRED: __

     

    ·         OSW-C Certification

    ·         Psychotherapy Experience
