    Job Description

    The SOCIAL WORK COORDINATOR reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

    RESPONSIBILITIES:

    • Assists in arranging non complex post discharge care by collecting and transmitting pertinent patient demographic and clinical information required to complete planning for post discharge care of placement, as directed.
    • Provides timely electronic updates of the appropriate information to accurately reflect the status of each referral request.
    • Provide appropriate provider referral information to the Social Workers enabling them to re evaluate and redirect the current patient plan of care in order to streamline service delivery.
    • Acts as resource to NMH staff by providing feedback as needed regarding post discharge provider guidelines, issues, and determination discovered during communication with providers.
    • Ensures post discharge provider and customer satisfaction by utilizing effective communication and interpersonal skills.
    • Provides entry, tracking, and trending of referral data and outcomes, as directed.
    • Participates in Performance Improvement initiatives and performs other functions as directed.
    • Fields community calls regarding outpatient needs.
    • Upholds the Northwestern Memorial Hospital policy on patient confidentiality.

  • Qualifications

    Qualifications

    REQUIRED:

    • High school diploma.
    • Computer skills to efficiently and effectively manage electronic systems.
    • Interpersonal and communication skills to effectively deal with a variety of people, including physicians, hospital leaders, nursing staff, patients, and family members.

    PREFERRED:

    • Working knowledge of post discharge care, such as an awareness of home care, nursing home, IV Infusion companies, and care facilities.
