    Job Description The Pharmacy Intern reflects the mission, vision, and values of NM, adheres to the organizations Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards. The Pharmacy Intern will work under the direction and supervision of a Licensed Pharmacist. Pharmacy Interns assist in the practice of pharmacy and perform all delegated functions. Description - Completes all functions associated with medication histories for patients - Performs outpatient prescription fill history review and Illinois PMP review - Obtains medication list and clarifies discrepancies through appropriate avenues such as patients, family, caregivers, and outpatient pharmacies - Documents current medication list in the electronic medical record including drug name, strength, frequency, indication, last dose, and preferred pharmacy - Performs medication reconciliation after each medicationhistory - Documents medication reconciliation in the electronic medical record and saves for pharmacist review prior to submission - With assistance from the supervising pharmacist, identifies patients who require medication education - Performs medication education and documents education in the electronic medical record - Assists with nursing inquiries by answering them if able or directing them to the appropriate resource - Participates in a mentorship program fostering career planning and residency preparation AA/EOE
