The Manager is responsible for the oversight and management of the Post-Acute Network. This position is a key partner to NMPN, NM Hospitals, Medical Groups, Ambulatory Care Coordination, and Community Partners. This role has responsibility for appropriate use of resources including staff recruitment and retention, human resource management, budgeting activities and ensuring financial control of assigned cost center(s).
The Manager coordinates all regional strategic and operational initiatives for the Post-Acute Network providers with emphasis on continuous improvement. This position assists the Director and, at times, leads activities for Strategic NMHC projects. The Manager will be a part of NMHC’s Collaborative Care & Outreach team and will provide leadership in developing a high-performing team that incorporates leadership principles and vision in performing operational functions.
RESPONSIBILITIES:
Manages and optimizes NMs Post-Acute Network ensuring that all strategic and operational initiatives meet the needs of NMs population while achieving quality aims, strategic goals, and clinical outcomes set by NM.
Drives the quality of care for the Post-Acute Network and is accountable for outcomes including, but not limited to readmissions, length of stay, timeliness of care, and utilization of the network.
Partners with teams across the NM system and Third-Party Vendors to implement education, care coordination, care pathways, information technology, and data needed to enable post-acute care teams to successfully provide care to NM patients.
Works collaboratively with the care coordination teams to ensure that NM patients have access to appropriate levels of Post-Acute care services and that these services meet the quality standards set forth by NM.
Facilitates ongoing communication with the Post-Acute Network along with our system partners to share data & quality metrics to support continuous improvement. This includes site visits within the Post-Acute Network.
Evaluates opportunities for and advises senior leadership on partnerships, and collaboration opportunities in the post-acute space.
Lead activities including project proposal/charter definition, budget development, resource allocation and management, information distribution and meeting facilitation as well as a comprehensive planning effort necessary to establish, measure, enhance and report organizational management performance.
Works closely with all relevant stakeholders to set program goals, clearly define operational dependencies, develop appropriate performance standards, metrics and reporting tools required for successful program design.
Demonstrate the ability to deal with the changing environment and to lead/facilitate and manage change efficiently and effectively where appropriate.
Be responsible for understanding vendor contracts with NMHC to ensure vendor compliance to project schedules, contractual agreements, and compliance with NMHC standards.
Develops and maintains productive and cordial relationships with clients and team members, serving as a link between multiple parties and entities; effectively manages project risks, issues and problems that arise within the program and recommend mitigation strategies, as needed, to achieve the desired results.
Analyzes large data sets to identify program opportunities and provides performance updates for cross-departmental initiatives and executive oversight.
