Full-time opportunity: Friday, Saturday, Sunday OR Mon, Tues, Wed, Thurs.

• Provide in-home visits to patients in need of skilled nursing care

• Deliver professional, compassionate, and reliable care

• Collaborate with patients, families, and care teams to ensure the best outcomes

Qualifications:

• Active LPN license in [State]

• Previous experience in home health or patient care preferred, but not required

• Strong communication and documentation skills

• Computer literate

• Dependable, compassionate, and patient-focused
