About the Role:

Join Skilled Nursing Pharmacy as a Nurse Consultant and make a meaningful impact in the lives of our patients. In this pivotal role, you will collaborate with healthcare teams to enhance patient care and optimize medication management.

Responsibilities:

Conduct comprehensive medication reviews for skilled nursing facilities.

Provide clinical consultation to nursing staff and healthcare providers.

Develop and implement individualized patient care plans.

Educate staff on best practices for medication administration and safety.

Monitor patient outcomes and recommend adjustments to therapy.

Participate in interdisciplinary team meetings to enhance care delivery.

Stay updated on industry regulations and ensure compliance.

Assist in training and mentoring new nursing staff on medication protocols.

Requirements:

Current RN license in Washington State.

Minimum of 3 years of nursing experience, preferably in skilled nursing or long-term care.

Strong knowledge of pharmacology and medication management.

Excellent communication and interpersonal skills.

Ability to work collaboratively within a healthcare team.

Detail-oriented with strong organizational skills.

Experience in clinical consulting or education is a plus.

Proficient in electronic health records and documentation.
