Phix Health is looking for an energetic MSN or BSN prepared RN who would like to complement our state of the art Chronic Care Primary Care organization. Our clinical specialty focuses on providing an integrated holistic care process to our clients that includes recognizing their emotional and mental health needs.

Minimum Requirements–RN with license in good standing, preferably BSN, MSN

Preferable – experience in long term care settings, familiarity with using electronic health records, knowledge of WORD and EXCEL

This position:

· Assists the provider with chart preparation including medication reconciliations.

· Reviews changes in care conditions, labs, and referrals.

· Completion and updating of validated assessment tools.

· Creating and Updating client care plans

· Obtains prior approvals

· Works with designated providers to meet their client needs.

· Other provider and client needs as they present.

Currently this position is scheduled for 3 days per week.
