    Nurse-Family Partnership (NFP) is an evidence-based maternal and early childhood home visiting program dedicated to improving outcomes for first-time parents and their children. Through ongoing nurse home visits beginning in pregnancy and continuing until the child turns two, we empower families with the education, support, and resources needed to build healthy futures. As a Nurse Home Visitor, you will provide direct, relationship-based care to first-time parents in a community setting. This role blends clinical expertise with coaching, education, and care coordination, while maintaining fidelity to the NFP model. You’ll collaborate with a supportive team, contribute to community partnerships, and help drive meaningful, long-term impact for families. Responsibilities: • Utilize the nursing process to assess needs, develop care plans, and support positive maternal and child health outcomes • Provide in-home visits in alignment with the Nurse-Family Partnership (NFP) model, managing a caseload of up to 30 families • Deliver health education, parenting support, and connect families with appropriate community resources • Support program outreach, community engagement, and continuous quality improvement initiatives • Participate in regular supervision, case conferences, and ongoing professional development Qualifications: • CPR certification (or ability to obtain) • Excellent communication, documentation, and organizational skills • Bachelor of Science in Nursing (BSN) required • Current Registered Nurse (RN) Indiana license in good standing (state-specific) • Understanding of, or willingness to learn, strengths-based and client-centered care approaches Compensation: $65,000
