Nurse-Family Partnership (NFP) is an evidence-based program focused on improving outcomes for first-time parents and their children. Through regular nurse home visits starting during pregnancy and continuing until the child’s second birthday, we partner with families to provide education, guidance, and support that fosters lifelong health and stability. As a Nurse Home Visitor, you will deliver compassionate, relationship-centered care to first-time parents in their homes and communities. This unique role combines clinical nursing practice with coaching, education, and care coordination. You’ll play a vital role in empowering families while working within the NFP model and collaborating with a multidisciplinary team dedicated to long-term impact. Responsibilities: • Apply the nursing process (assessment, diagnosis, planning, intervention, evaluation) to support maternal and child health outcomes • Establish trusting, therapeutic relationships using motivational interviewing and strengths-based, client-centered techniques • Assess physical, emotional, social, and environmental needs and develop individualized care plans • Maintain thorough, timely documentation in the NFP data system and use data to inform practice • Provide health education, parenting guidance, and referrals to community resources Qualifications: • Valid driver’s license, reliable transportation, and ability to travel locally for home visits • Active Registered Nurse (RN) Indiana license in good standing (state-specific) • Demonstrated ability to build rapport while maintaining professional boundaries • CPR certification (or ability to obtain) • Proficiency in Microsoft Office and comfort learning new systems Compensation: $65,000
