Licensed Social Worker Needed in Concord, MA

Weekly pay: $1650.00/week

Number of hours: 40 guaranteed

Temp to perm option

The Care Management Department is looking for a Licensed Social Worker to work 40 hours Monday thru Friday, one weekend per month (day off during the week in exchange). Shift hours are 8-4:30 but can flex a bit.

LSW should have experience in the management of complex medical patients, palliative and hospice care, SDOH, and general emotional support. They should understand discharge planning and work with a multi-disciplinary team. Experience in Meditech and/or All scripts is a benefit.

This would be a 13-week assignment with the potential for extension if we have not successfully
hired into the role

Job Duties of job title:

• Ability to project a professional image
• Knowledge of regulatory standards and compliance requirements
• Working knowledge and ability to apply professional standards of practice in job situations
• Strong organizational and analytical skills
• Working knowledge of computer and software applications used in job functions (Order entry, data input)

Education:

• Graduated with a BSN in Social Work
• Must be a licensed social worker
