Home Health RN/LPN – Job Description

Job Summary We are seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Home Health Team. Full-time, Part-time, and PRN positions. In this role, you will provide high‑quality, patient‑centered care to an adult or a pediatric client in the comfort of their home. The ideal candidate brings strong clinical skills, physiology knowledge, and experience in hospital, skilled nursing, home care, or community care settings.

Responsibilities

• Work12‑hour shifts, including overnights and weekends as needed. • Provide direct skilled nursing care to adult and pediatric clients in their homes. • Administer medications, monitor vital signs, and assist with daily living activities. • Develop and implement individualized care plans to ensure safe, effective, and patient‑centered care. • Collaborate with physicians, nurses, and interdisciplinary team members to support comprehensive care coordination. • Document all care accurately using electronic medical records (EMRs). • Utilize WellSky EMR (experience preferred; training provided). • Maintain timely and accurate clinical documentation, including progress notes and discharge summaries.

Requirements

• Current Florida RN or LPN license. • Experience in a hospital, skilled nursing, or home/community care setting. • Strong physiology knowledge and experience with airway management. • Ability to travel to patients’ homes and provide care. • Ability to work effectively in a fast‑paced environment while delivering high‑quality patient care.
