_ A Day in the Life_

• The Stroke Nurse Navigator will support patients throughout the healthcare process, from admission and diagnosis, through treatment, and to discharge and 90 days post discharge.'

• In addition to direct contact patient care, the Stroke Nurse Navigator will work closely with the Stroke Program Manager, Neuroscience service line Executive Director, and the entire stroke team to assure provision of quality care Stroke Patient care, identify opportunities for improvement, and support ongoing performance improvement initiatives.

• Serves in an expanded health care role and will provide resources, education and assistance with accessing clinical and supportive care services offered within the Munson Health Care system and surrounding communities

• Support quality improvement initiatives and adherence to safety standards.

Qualifications

_ What’s Required _

• Licensed RN in the State of Michigan

• Bachelor of Science degree in nursing Required

• A minimum of 2 years of Neuro / Stroke nursing

• Nursing Stroke Certification Preferred (Required to obtain within 2 years)
