Position Description:

We are looking for a dedicated Nurse Practitioner ( NP ) in Jersey City, NJ for our Skilled Nursing Facility ( SNF ) for bedside patient care. This is a full time, in person position, Monday - Friday 9am - 5pm.

Qualifications:

• Valid Nurse Practitioner ( NP ) license in the state of New Jersey
• 1 - 2 years experience of Nurse Practitioner ( NP ) experience
• Active DEA License
• Nursing home and ICU experience preferred

Responsibilities:

• Upon admission to the facility, examine residents and their medical records.
• Check ups, management of chronic conditions and medications, transitional care management, discharge planning and family engagement.
