    ORGANIZATION OVERVIEW

    Across The Life Span, PLLC (ATLS) is an integrated behavioral health, primary care, and opioid addiction treatment organization serving Caswell County and surrounding communities in North Carolina under NC Medicaid. Our mission is to provide compassionate, whole-person care across all stages of life, ensuring equitable access to high-quality clinical services for our patients.

    ATLS operates multiple clinical sites, including our Greensboro and Burlington locations, and is committed to growing our team of skilled clinicians to meet rising community need.

    POSITION SUMMARY

    The Nurse Practitioner (NP) will provide comprehensive, evidence-based clinical care across primary care and psychiatric health disciplines, with specialized competency in osteoarthritis (OA) joint injection therapy. This dual-site position serves patients at both the Greensboro and Burlington locations, delivering patient-centered services within a collaborative, multidisciplinary care model.

    The NP functions with a high degree of clinical autonomy while collaborating closely with the Medical Director, psychiatric providers, and the broader clinical team to ensure continuity of care, compliance with NC Medicaid standards, and adherence to ATLS quality improvement protocols.

    KEY RESPONSIBILITIES

    Primary Care Services

    Conduct comprehensive and episodic patient assessments including history, physical examination, and diagnostic evaluation

    Order, interpret, and manage diagnostic testing including labs, imaging, and specialty referrals

    Diagnose and manage acute, chronic, and complex medical conditions across the lifespan

    Develop, implement, and monitor individualized care plans in alignment with evidence-based clinical guidelines

    Prescribe medications, therapies, and treatments in compliance with North Carolina Board of Nursing and NC Medical Board prescriptive authority regulations

    Provide chronic disease management for conditions including diabetes, hypertension, COPD, asthma, and metabolic disorders

    Conduct annual wellness visits, preventive screenings, and immunizations per CDC and USPSTF guidelines

    Coordinate referrals to specialists, community resources, and ancillary services

    Osteoarthritis Injection Therapy

    Perform joint aspiration and corticosteroid injections for osteoarthritis of the knee, shoulder, hip, and other affected joints

    Administer hyaluronic acid (viscosupplementation) injections per established clinical protocols

    Evaluate patient candidacy for injection therapy, including review of imaging and prior treatment history

    Educate patients on procedural expectations, post-injection care, activity modifications, and expected outcomes

    Document all injection procedures in compliance with NC Medicaid billing requirements and ATLS quality standards

    Monitor injection outcomes and adjust treatment plans based on patient response and functional improvement goals

    Stay current with emerging evidence and best practices for OA interventional pain management

    Psychiatric Health Services

    Provide psychiatric evaluation and medication management for patients presenting with depression, anxiety, PTSD, ADHD, bipolar disorder, and related conditions

    Conduct mental health assessments including standardized screening tools (PHQ-9, GAD-7, PCL-5, etc.) in alignment with ATLS clinical protocols

    Prescribe and monitor psychotropic medications in accordance with prescriptive authority and NC practice standards

    Collaborate with licensed clinical staff, including the Clinical Services Manager and behavioral health counselors, to ensure integrated care delivery

    Support opioid addiction treatment (OAT) patients through co-occurring mental health management and coordination with MAT program staff

    Participate in care coordination for patients with complex psychiatric and medical comorbidities

    Documentation & Compliance

    Maintain accurate, timely, and complete clinical documentation in the electronic health record (EHR) in compliance with NC Medicaid, HIPAA, and ATLS policies

    Support completion of Comprehensive Clinical Assessments (CCAs), Psychiatric Care Plans (PCPs), and service authorization requests as required

    Participate in internal audits, peer review, and quality improvement initiatives as directed by the QA/QI Officer

    Adhere to ATLS two-level document review workflows and all applicable regulatory compliance standards

    Fulfill mandatory reporting obligations and maintain current knowledge of applicable federal and state regulations

    Organizational Contribution

    Participate in multidisciplinary team meetings, case conferences, and clinical staff training activities

    Contribute to orientation of new clinical staff as requested

    Maintain continuing education and professional development requirements per licensure standards

    Perform other duties as assigned by the Medical Director or CEO

    MINIMUM QUALIFICATIONS

    Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program

    Current, unrestricted Nurse Practitioner licensure in the State of North Carolina

    National board certification as Family Nurse Practitioner (FNP-BC, FNP-C) or Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) — dual certification preferred

    Current DEA registration and prescriptive authority in North Carolina

    Current BLS/CPR certification (ACLS preferred)

    Minimum of two (2) years of clinical NP experience in primary care, behavioral health, or integrated care settings

    Demonstrated competency or willingness to obtain training in joint injection therapy

    Experience with NC Medicaid billing, documentation, and compliance standards

    PREFERRED QUALIFICATIONS

    Experience in opioid addiction treatment (MAT/OAT) settings

    Proficiency with, NC Tracks, or similar EHR/billing platforms

    Bilingual or multilingual abilities to serve diverse patient populations a plus

    Experience conducting osteoarthritis joint injections or completion of formal injection therapy training

    Knowledge of NC Medicaid value-based care and quality reporting requirements

    WORK ENVIRONMENT & CONDITIONS

    This position requires regular travel between the Greensboro and Burlington clinic locations. Schedule may include variable hours to meet patient demand.
