Position Summary:
Commonshare is seeking a compassionate and experienced Advanced Nurse Practitioner (ANP) to deliver comprehensive, patient-centered primary care services to elderly residents at Imperial Plaza, a senior living community in Richmond, Virginia. This on-site role will focus on providing accessible, high-quality healthcare tailored to the complex needs of a geriatric population, supporting aging in place and reducing unnecessary hospitalizations.
Responsibilities:
Perform physical exams, assessments, and routine primary care visits directly at the patient's residence within Imperial Plaza.
Diagnose and manage acute and chronic health conditions in older adults, including but not limited to diabetes, hypertension, COPD, and dementia.
Develop and implement personalized care plans in collaboration with patients, families, pharmacists, and other healthcare providers.
Monitor and adjust treatment plans based on patient outcomes and clinical guidelines.
Provide education to residents and their families about disease prevention, medication adherence, and wellness.
Coordinate care with Commonshare Pharmacy and other healthcare partners for medication management and follow-up.
Facilitate referrals to specialists and community resources when necessary.
Maintain detailed and compliant electronic health records and documentation.
Participate in interdisciplinary care team meetings and contribute to overall program development.
Qualifications:
Current license as a Nurse Practitioner (NP) in the state of Virginia.
Certification in Adult-Gerontology Primary Care or Family Nurse Practitioner (AGPCNP or FNP) strongly preferred.
Minimum of 2 years' experience in geriatric, internal medicine, or primary care settings.
Strong clinical assessment skills and comfort with managing multiple chronic conditions in older adults.
Excellent communication and interpersonal skills with a patient-centered approach.
Experience working in residential or long-term care settings is a plus.
