About the Role:
- Nurse Practitioner
- $140/visit
- Travel: Metro Oregon Traveler. Covers Washington, Clackamas (Hillsboro and Oregon City areas)
- In-home, local travel
- You choose your hours (min. 24/week). Just assessments, no meds, no Rx, no clinic.
What You'll Do:
- Complete 6–9 in-home health assessments/day
- Review of medical history and risk factors
- Collect vitals, labs, and screening data
- Document findings in EMR
- Educate members on next steps
- Coordinate follow-up if red flags show
- Travel across Metro Oregon (Washington, Clackamas - Hillsboro and Oregon City areas)
Who You Are:
- Must have license/certification: APRN, Certified Nurse Practitioner
- Must be registered with Medicare and/or Medicaid
- Valid driver's license + reliable car
- Able to work independently in the field
