    Part-Time (Weekends | Friday–Sunday) Fee for Service compensation Rates discussed during the interview based on availability and coverage needs.

    Job Summary

    We are seeking a Nurse Practitioner to join our growing skilled nursing facility practice. This role is well-suited for early- to mid-career clinicians looking for meaningful work, flexible scheduling, and a collaborative clinical environment.

    You will provide comprehensive medical care to residents in skilled nursing facilities, work closely with interdisciplinary teams, and contribute to high-quality, patient-centered care. This position offers autonomy, consistency, and the opportunity to build strong clinical relationships.

    Responsibilities – Skilled Nursing Facility Provider

    Provide assessment, diagnosis, and management of acute and chronic conditions

    Conduct regular patient rounding and timely evaluations

    Order and interpret labs, diagnostics, and imaging

    Prescribe and manage medications in compliance with regulations

    Maintain accurate, timely documentation in the EMR

    Collaborate with nursing, rehab, pharmacy, hospice, and administrative teams

    Respond to change-in-condition and urgent care needs

    Support advance care planning and end-of-life discussions

    Participate in quality assurance and regulatory readiness efforts

    Qualifications

    Active Pennsylvania Nurse Practitioner license and required certifications

    Graduation from an accredited Nurse Practitioner program

    Strong attention to detail and timely documentation practices

    Self-motivated, dependable, and patient-centered

    Willingness to participate in a rotating on-call schedule
