Position Title: Nurse Practitioner (NP) / Physician Assistant (PA) – Skilled Nursing Facility Primary Care Provider

Position Summary We are seeking a motivated and compassionate NP or PA to serve as the primary care provider for a panel of patients in our partner SNFs. The role focuses on comprehensive patient management, proactive care planning, and close collaboration with nursing staff, therapy teams, and attending physicians. This is an excellent opportunity to practice longitudinal medicine in a supportive, team-based environment while making a measurable impact on patient outcomes.
Key Responsibilities
Perform daily rounding in assigned SNFs, acting as PCP for long-term care and post-acute patients
Conduct comprehensive history and physical exams, manage chronic conditions, and perform regulatory visits (30/60/90 day)
Address acute changes in condition promptly, including evaluation of new symptoms and initiation of treatment
Collaborate with DON, therapy, social services, and facility staff to ensure care plan compliance and regulatory readiness
Participate in weekly IDT meetings, QAPI initiatives, and family care conferences as needed
Document encounters in EMR (PointClickCare/Practitioner Workspace) in a timely and compliant manner
Support facility quality measures including reducing readmissions, falls, and inappropriate polypharmacy
Communicate with attending physicians and supervising medical director to escalate care when needed
Qualifications
Active NP or PA license in the state of practice (Florida license required or eligibility to obtain)
Board certification in Adult, Family, or Geriatric practice (preferred for NPs)
Experience in geriatrics, internal medicine, or post-acute care preferred (new grads with strong interest considered)
Strong communication skills and ability to work collaboratively with facility teams
Familiarity with EMRs (PointClickCare experience a plus)
