Position Summary: We are seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join our expanding Primary and Urgent Care team. This is an exciting opportunity to work in a collaborative outpatient setting where you'll make a meaningful impact on patient health and outcomes.

Key Responsibilities:

• Evaluate, diagnose, and manage both acute and chronic medical conditions
• Perform physical exams and wellness screenings
• Order and interpret diagnostic tests, labs, and imaging
• Create and manage treatment plans, including prescribing medications
• Coordinate care and refer patients to specialists as needed
• Accurately document patient encounters in the Electronic Medical Record (EMR) system
• Work closely with physicians, nurses, and other members of the care team

Qualifications:

• NP: Master’s in Nursing (APRN), active NY State Nurse Practitioner license, and board certification
• PA: Graduate of an accredited PA program, active NY State license, and NCCPA certification
• Valid DEA registration and prescriptive authority
• 1–2 years of experience in primary or urgent care preferred
• Bilingual in Spanish strongly preferred
• Excellent communication, clinical judgment, and patient-centered care approach
