Job Description: Nurse Practitioner - Nephrology
Bluegrass Kidney Consultants is seeking a highly skilled and compassionate Nurse Practitioner specializing in Nephrology to join our dedicated team. The Nurse Practitioner will provide primary and specialized care to patients both hospitalized and in the office, suffering from kidney diseases and disorders, working closely with our team of nephrologists and other healthcare professionals.
Responsibilities:
1. Assess and diagnose patients with kidney diseases or disorders through comprehensive history-taking, physical examinations, and ordering and interpreting laboratory and diagnostic tests.
2. Develop and implement individualized treatment plans for patients, including prescribing medications, administering therapeutic interventions, and providing counseling on lifestyle modifications.
3. Collaborate with nephrologists and other healthcare professionals to provide integrated and comprehensive care to patients with kidney-related conditions.
4. Assist in the management and coordination of care for patients undergoing dialysis, including providing education and counseling on dialysis procedures, monitoring dialysis treatments, and managing any acute complications or adverse events.
5. Perform routine procedures such as catheter insertion, wound management, and administration of intravenous medications under appropriate supervision and in compliance with established protocols.
6. Educate patients and their families on kidney diseases, treatment options, and self-management strategies, emphasizing the importance of adherence to medical regimens and lifestyle modifications.
7. Conduct follow-up visits with patients to assess treatment effectiveness, monitor disease progression, and address any complications or concerns.
8. Act as an advocate for patients, ensuring their needs and preferences are respected and incorporating shared decision-making into the care planning process.
9. Maintain accurate and complete patient records, documenting all assessments, interventions, and outcomes in accordance with established standards and regulatory requirements.
10. Stay updated with current developments and best practices in nephrology, participating in continuing education activities, conferences, and professional organizations to enhance knowledge and skills.
Requirements:
1. Master's degree in Nursing with specialization as a Nurse Practitioner in Adult or Family Health.
2. Current and unrestricted state nursing license as a Nurse Practitioner.
3. Current National Certification as a Nurse Practitioner.
4. Minimum of 2 years of clinical experience as a Registered Nurse (RN) in ICU/CCU setting
5. Strong clinical skills in assessing, diagnosing, and managing kidney diseases and disorders.
6. Excellent communication and interpersonal skills, with the ability to establish rapport with patients, families, and colleagues.
7. Proficiency in using electronic medical record systems and other healthcare technologies.
8. Ability to work effectively in a multidisciplinary team and collaborate with other healthcare professionals to deliver high-quality care.
9. Commitment to evidence-based practice, quality improvement, and ongoing professional development.
