We are seeking a motivated and skilled mid-level provider to join our outpatient endocrinology team serving South Riverside County. In this role, you will take a lead role in the assessment, treatment, and ongoing management of patients with endocrine and metabolic disorders — including diabetes, thyroid disease, adrenal conditions, and other hormonal pathologies.
You will work within a multidisciplinary team environment, rounding with a degree of clinical autonomy while collaborating closely with physicians, dietitians, and nursing staff to ensure comprehensive, coordinated patient care. Candidates with prior acute or hospital-based experience are strongly encouraged to apply.
Key Responsibilities
Clinical Assessment Perform comprehensive endocrine evaluations including detailed history, physical examination, laboratory interpretation, and risk stratification for diabetes and hormonal disorders.
Treatment & Disease Management Develop individualized, evidence-based treatment plans for conditions including diabetes mellitus (Type 1 and Type 2), thyroid disorders, osteoporosis, adrenal and pituitary conditions, and metabolic syndrome.
Patient Education Empower patients and their families through education on disease processes, lifestyle modifications, medication adherence, and self-management strategies.
Collaborative Care Partner with physicians, dietitians, nurses, and other specialists to coordinate care, optimize outcomes, and support a team-first culture.
Procedures & Documentation Perform and interpret point-of-care testing (glucose monitoring, A1C, thyroid ultrasound if trained); adjust medications accordingly. Maintain thorough, accurate clinical documentation including assessments, treatment plans, progress notes, and outcomes.
Quality & Advocacy Participate in quality improvement initiatives and advocate fiercely for each patient's access to comprehensive, compassionate care.
Qualifications
Education & Certification
Master's degree in Nursing with Nurse Practitioner certification required
Board certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) preferred
Physician Assistant candidates with equivalent credentials will be considered
Licensure
Current California NP or PA license
Active DEA registration
APRN certification (for NP candidates)
Experience
Minimum 1 year of clinical experience as a Nurse Practitioner or Physician Assistant
Prior exposure to endocrinology, diabetes management, or internal medicine strongly preferred
At least 1 year of acute or hospital-based experience preferred
Skills & Attributes
Strong clinical decision-making and critical thinking skills for managing complex endocrine presentations

Comfortable working autonomously within a collaborative team structure
Genuine commitment to patient-centered, compassionate care
Certified Diabetes Care and Education Specialist (CDCES/CDE) or endocrinology-focused training a plus, but not required
