What You'll Do
As a Post-Acute Care Provider, you will:
Provide comprehensive primary care services in skilled nursing and post-acute care facilities.
Evaluate, diagnose, and manage acute and chronic medical conditions.
Develop individualized treatment plans that improve function, mobility, and quality of life.
Perform routine follow-up visits, admissions, discharge planning, and transitional care.
Collaborate closely with nursing staff, therapists, specialists, and facility leadership.
Educate patients and families regarding disease management and preventive care.
Maintain accurate and timely documentation using electronic health records.
Qualifications
We're looking for providers who have:
Current Nurse Practitioner or Physician Assistant license in Florida
National Board Certification
DEA license (or ability to obtain)
Excellent clinical judgment and communication skills
Experience in Internal Medicine, Family Medicine, Geriatrics, Hospital Medicine, Emergency Medicine, or Post-Acute Care preferred
Skilled Nursing Facility experience is a plus, but not required
Commitment to compassionate, patient-centered care
