    Join a modern, relationship-driven concierge primary care practice in Mequon, WI, seeking a Physician Assistant or Nurse Practitioner to grow with the team. This is a full-time opportunity offering a personalized care model, strong collaboration, and a high-touch patient experience in a supportive, low-volume environment.


    Responsibilities:

    • Provide comprehensive primary care including diagnosis, treatment, and chronic disease management
    • Conduct in-depth, personalized visits focused on education and preventive health planning
    • Order and interpret labs, imaging, and diagnostic tests
    • Develop individualized care plans aligned with patient goals and lifestyle
    • Perform in-office procedures such as injections and wound care
    • Deliver concierge-level service with same-day access, proactive follow-up, and care coordination
    • Collaborate closely with physicians and care team to ensure seamless patient experience

    • 2–3 years of primary care experience preferred (concierge experience a plus)


    Schedule:

    4 days in clinic + 1 administrative day (Monday–Thursday: 8:00 AM – 5:00 PM)
