We’re hiring a Primary Care Nurse Practitioner to join a new clinic opening in Idaho Falls. This is a unique opportunity to help shape a growing, NP-led healthcare organization focused on value-based care for older adults in underserved communities.

You’ll provide full-scope primary care for a panel of adult and senior patients, supported by a Clinic Manager, Medical Assistant or LPN, and collaborative MD advisor. With an emphasis on prevention, chronic condition management, and coordinated care, you’ll have the time and resources to truly focus on your patients.

Responsibilities:

• Deliver comprehensive, holistic primary care in an outpatient setting
• Manage a panel of Medicare Advantage and senior patients
• Conduct wellness visits, health risk assessments, and chronic disease management
• Coordinate care with internal and external healthcare partners
• Participate in quality improvement and clinical team meetings

Requirements:

• Active FNP or AGNP license in Idaho
• DEA license
• CPR/BLS certification
