    About the Role

    We’re seeking a Certified Registered Nurse Practitioner (CRNP) to provide compassionate, high-quality primary care to patients across the lifespan—from infants to older adults. This role focuses on preventive care, chronic disease management, and treating acute conditions in a collaborative, team-based environment.

    What You’ll Do

    Provide primary care using evidence-based guidelines

    Perform exams, diagnose conditions, and manage treatment plans

    Deliver preventive care, immunizations, and patient education

    Manage chronic and acute health conditions

    Follow up on labs, imaging, and specialty referrals

    Collaborate with physicians and care teams

    Participate in case reviews, quality initiatives, and community outreach

    Maintain accurate, timely documentation and ensure HIPAA compliance

    Schedule & Expectations

    Full-time role with at least one evening per week

    Shared on-call coverage (weekends/holidays)

    Team-oriented, mission-driven work environment

    What You Bring

    Master’s degree in Advanced Practice Nursing

    Current or eligible PA CRNP and RN licenses

    Prescriptive authority eligibility in Pennsylvania

    CPR certification

    Eligible for insurance credentialing (Medicare/Medicaid/commercial)

    Experience in primary care or community health (preferred)

    Bilingual English/Spanish preferred
