We’re seeking Nurse Practitioners to perform in-home Annual Wellness Visits for Medicare members. These visits are conversational, consultative, and education-focused— no prescribing, no treatment plans.

What You’llDo

• Conduct comprehensive in-home wellness assessments
• Educate members on preventive care benefits
  
• Identify potential gaps in care to help reduce avoidable ER visits and hospitalizations
• Complete documentation using provided EMR tools
• Visits are face-to-face and take place where members live—allowing for more personalized, impactful care.
  

Requirements

• Active State NP license and Board Certification
• Valid NPI 
• Minimum 3 months of wellness exam experience preferred
• Comfort with EMR/documentation systems
• Ability to travel within a region (visits are clustered in a 10 mile radius)
• Bilingual/Spanish Speaking a plus
• Give on average 2 full days per week.
