    What You'll Be Doing

    The Nurse Practitioner (NP) will be responsible for providing comprehensive care to patients in a single skilled nursing facility. This is a Monday–Friday role with no night or weekend call with a high-degree of flexibility to accommodate the demands of life. It's designed for self-motivated clinicians who want to focus on delivering high-quality, efficient, and patient-centered care. You will collaborate with the facility staff, physicians, and other members of the healthcare team to ensure timely, effective, and coordinated care.

    Key Responsibilities

    • Provide direct medical care to patients in the assigned facility, including assessment, diagnosis, treatment, and follow-up.

    • Manage acute and chronic conditions, focusing on high-quality, efficient care delivery.

    • Coordinate with facility staff, specialists, and families to develop and update individualized care plans.

    • Support transitions of care, including post-hospital evaluations, medication reconciliation, and patient/family education.

    • Maintain thorough, timely, and compliant documentation of patient encounters and care plans.

    • Participate in quality initiatives and process improvements that enhance patient outcomes and efficiency.

    • Collaborate with the interdisciplinary team to ensure continuity of care and adherence to evidence-based protocols.

    Skills & Abilities

    • Strong clinical knowledge of adult and geriatric medicine, with expertise in chronic disease management.

    • Excellent communication and collaboration skills with patients, families, and facility staff.

    • Ability to manage a patient panel efficiently while maintaining a high standard of care.

    • Strong organizational and documentation skills.

    • Commitment to patient-centered, evidence-based practice.

    Qualifications

    • Current Nurse Practitioner license in the state of Georgia.

    • Certification in Internal Medicine, Geriatrics, or Family Practice required.

    • Must have an unrestricted DEA license

    • Minimum 1–2 years of NP experience preferred; experience in post-acute, SNF, or geriatrics strongly preferred.

    • Ability to work independently in a facility setting with physician support.

    • Familiarity with EHR systems and clinical documentation best practices.
