Job Summary
Seeking a dedicated Nurse Practitioner (NP) to join our interdisciplinary behavioral health team. Temah Healthcare Services, LLC is a growing, patient-centered healthcare organization dedicated to providing integrated primary and behavioral health services to our community. We are seeking a skilled and motivated (Entry CRNP) Nurse Practitioner to deliver comprehensive and high-quality care.
This role is ideal for a provider passionate about holistic, community-based care and improving access to mental health services within the public health system.
Responsibilities
Conduct comprehensive mental health evaluations and psychiatric assessments
Perform risk assessments and crisis intervention
Develop and manage individualized treatment plans
Provide medication management and follow-up care as appropriate
Collaborate with peer specialists, case managers, and behavioral health professionals
Maintain accurate documentation in electronic health record (EHR) systems
Participate in community-based outreach and public health initiatives
Provide education and support to patients and families regarding treatment plans
Ensure compliance with all state regulations and clinical standards
Qualifications
Active Nurse Practitioner license in the state of Maryland
Current unrestricted license to practice
Experience in behavioral health, psychiatric care, or community health
Conduct comprehensive health assessments, including history and physical examinations • Diagnose and manage acute and chronic medical conditions • Develop and implement individualized treatment plans • Order and interpret diagnostic tests and laboratory results • Prescribe medications, including psychotropic medications, in accordance with Maryland regulations • Perform routine wellness exams, preventive screenings, immunizations, and health promotion education • Deliver integrated behavioral health services including screening, diagnosis, and management of depression, anxiety, substance use disorders, and other psychiatric conditions • Conduct brief interventions, crisis management, and motivational interviewing as appropriate • Collaborate with psychiatrists, therapists, and social workers for coordinated care • Maintain accurate and timely documentation in the electronic health record (EHR) • Ensure compliance with HIPAA and all federal, state, and local regulations • Participate in quality improvement initiatives
Strong communication and documentation skills
Ability to work collaboratively within multidisciplinary teams
Requirements
Minimum 25 hours per week availability
Proficiency in MS Office and EHR systems
Excellent organizational and time-management skills
Ability to work in-office and within community settings
Preferred Experience
Community-based or public health programs
Behavioral health services in clinical or outreach settings
Health education or wraparound support services
