Location: Brevard County, FL

Schedule: 1–2 days per week (in-person only) Compensation: $75–$90/hour, commensurate with experience

Position Summary:

Evolutionary Healthcare is seeking a compassionate and detail-oriented Nurse Practitioner (NP) to lead our Transitional Care Management (TCM) initiative in Brevard County. This role focuses on conducting in-person follow-up visits with patients recently discharged from hospitals or skilled nursing facilities. The ideal candidate will be experienced in managing medically complex patients and passionate about improving care transitions and reducing readmissions.

Key Responsibilities:

Perform in-person TCM visits in clinic, facility, or home settings within the 7–14 day window post-discharge

Conduct thorough assessments including medication reconciliation, care plan review, and symptom monitoring

Identify and address barriers to recovery and prevent avoidable hospital readmissions

Coordinate care with primary care physicians, specialists, and facility staff

Document all visits timely and accurately in the EMR for compliance and billing

Maintain adherence to Medicare’s TCM requirements including contact within two business days of discharge

Collaborate with internal teams to continuously improve care transition workflows

Qualifications:

Active and unrestricted Florida APRN license

Board certification in Adult-Gerontology, Family Practice, or Internal Medicine

Minimum 2 years of clinical experience in post-acute, internal medicine, or transitional care settings

Proficient in EMR documentation and Medicare compliance standards

Strong communication, critical thinking, and organizational skills

Comfortable traveling within Brevard County (mileage reimbursement provided)

Compensation & Schedule:

$75–$90/hour, based on experience and scope of responsibilities

1–2 days per week with flexible scheduling depending on patient volume

Paid mileage for travel within coverage area

Potential to expand into leadership or additional clinical programs
