Location: Midland, MI • Onsite at skilled nursing & long-term care facilities
Pay: $123,000–$135,000 base + 10% corporate bonus
Schedule: Mon–Fri, days • No on-call • No weekends
Team: Skilled nursing & long-term care partners

What You'll Do

• Provide care in nursing and long-term care facilities

• Manage chronic conditions, perform full assessments

• Close care gaps (HEDIS and STAR measures)

• Lead care planning with facility teams

• Support and educate patients' families

Must-Haves

• Active, unrestricted APN license in Michigan

• Board certified and Medicare credentialed

• 6+ months in skilled nursing or long-term care

• Comfortable with digital tools and team-based care

• Able to work onsite in Midland, MI

Perks & Pay

• Pay: $123K–$135K base + 10% corporate bonus
