  • Responsibilities

    Work directly with patients in their homes while keeping control of your schedule and patient relationships.

    • Role: Nurse Practitioner / Physician Assistant
    • Location/Type: Memphis, TN • Mobile / In-Home
    • Pay: $60/hour first 90 days → $120,000 salary after ramp
    • Schedule: Minimum 32 hours/week availability

    Own your patient panel and deliver full primary care in the home. Less clinic bureaucracy. Real patient relationships.

    What you'll do

    Deliver in-home primary care visits for adult patients

    Conduct physical exams and diagnose conditions

    Prescribe medications and adjust treatment plans

    Build personalized care plans for chronic conditions

    Coordinate care with specialists and healthcare partners

    Complete follow-ups after hospital discharge (TCM visits)

    Document visits and communicate with care team

    Must-haves

    MD, DO, Nurse Practitioner, or Physician Assistant license

    Licensed in Tennessee

    Master's degree in Nursing or Physician Assistant Studies

    Strong clinical assessment and diagnosis skills

    Authorized to work in the United States

    Comfortable working independently in patient homes

    Nice to have

    Experience in in-home or mobile healthcare

    Wound care certification or experience

    Experience with transitional care or chronic care management

    Perks & pay

    Pay: $60/hour ramp → $120,000 salary after full schedule

    Retention bonus: $2,500 quarterly ($10,000 annually)

    PTO: Up to 4 weeks annually

    CME allowance: $2,500 annually

    401(k) eligibility after 90 days

    Wound care bonus: 5% of net wound care revenue

    Overtime: $60/hour for work over 40 hours

    Mileage reimbursement after 100 miles/day
